“I’m just a social drinker. How bad can it be?”
That’s what Naman, 33, used to say. I first met him in Delhi while I was doing my DM in Hepatology at ILBS (Institute of Liver and Biliary Sciences) . His story stayed with me because it reflects what so many people go through. He had a corporate job, enjoyed weekend drinks with friends, and felt perfectly fine. Then one day, his friends noticed something unusual: his eyes had turned yellow. Routine office health checks had already shown mild liver abnormalities, but he brushed them off because he had “no symptoms.” A FibroScan finally revealed the truth: his liver had become stiff.
This is where MetALD, or metabolic dysfunction with alcohol-associated liver disease, comes in (Combination of metabolic-dysfunction associated steatotic liver disease, which was earlier known as Non alcoholic steatohepatitis or NASH; and Alcohol associated liver disease or ALD).
Even modest drinking (DRINKING CANNOT BE MODEST) can become harmful when combined with conditions like obesity, high blood pressure, diabetes, high cholesterol, or a strong family history (of sugar or pressure) . And here’s the catch for Indians: our BMI cut-offs are lower than Western standards, which means many of us are at risk much earlier than we think.
Here’s the hard truth: no amount of alcohol is completely safe for your liver, especially if you are overweight, diabetic, or already living with fatty liver disease.

The good news? Naman turned things around, and so can you. He cut back on alcohol, paid attention to his weight, and got regular screenings. Small steps made a big difference. Yes, CIRRHOSIS is REVERSIBLE. We will talk about cirrhosis in another post.
What is MetALD?
MetALD is what happens when two forces gang up on your liver: alcohol and metabolic risk factors. On their own, they can cause trouble. Together, they double the damage.
Drinking cut-offs in MetALD
Men: 30 to 60 grams of alcohol a day (about 2 to 4 drinks)
Women: 20 to 50 grams a day (about 1 to 3 drinks)
But if you already have obesity, diabetes, or cholesterol issues, even less can hurt your liver.
Indian BMI cut-offs
Overweight: 23 or more
Obese: 25 or more
That means many Indians, including people in Kerala, may reach the danger zone much earlier than they realize. For a 165cm tall man, 68 kgs would be overweight – THINK ABOUT IT
Disclaimer – BMI is not a good measure of obesity BUT AN INDICATION – its easily available. We will talk about other measures in another blog post.
How much alcohol is really safe?
The safest option is to avoid it altogether. Every sip makes your liver work harder, and when you already have risk factors, you’re stacking the odds against yourself.
